Note: A passport is *highly recommended* for U.S. visitors entering Canada. U.S. citizens should have their birth certificate or citizenship certificate as well as a picture ID; permanent residents (who are not citizens) need their alien-registration card and passport. Citizens from all countries other than the U.S. must have a valid passport, and in some cases a visitor visa may be required.

If registration and payment are not received by this date, you will be expected to pay the ON-SITE fee upon arrival at the conference.

Student registration is available for undergraduate and graduate students ONLY. The student rate is not available to post-doctorates.
The Conference Organizing Committee expects every speaker of a scheduled presentation to register and attend the conference.
Please Note: It is SIAM policy that all speakers must pre-register. Speakers who do not pre-register will be required to register on-site at the higher rate.
If it becomes necessary for a speaker to cancel a presentation, he or she must try to find an alternate presenter immediately, preferably a co-author. SIAM Program Manager, Connie Young, must be informed immediately of any change to a scheduled presentation. Please e-mail her at email@example.com. A “no-show” or cancelled presentation can cause serious inconvenience to the attendees and conference organizers. The committee thanks all speakers in advance for their compliance with this request.

To qualify for the pre-registration rate, return the Registration Form with payment to reach the SIAM office by Thursday, May 15, 2003.
Any registration received at the SIAM office after Thursday, May 15, 2003 will be subject to the full registration fee. We urge you to pre-register and save $60.
There will be no prorated fees. Refunds will not be issued after Thursday, May 15, 2003.
