Information submitted on this form will be used to determine eligibility for child care subsidies at the 2014 SIAM Annual Meeting, Chicago, Illinois, July 7-11, 2014. All child care arrangements need to be made by the family. Subsidies will be given in the form of reimbursement after the meeting.
