Recording Release & Authorization Agreement

Thank you very much for agreeing to share your knowledge, experience, and professional insights by participating in a SIAM conference. Your involvement helps SIAM fulfill its mission of advancing the knowledge of applied and computational mathematics. We appreciate your cooperation in reading and indicating your agreement with this form because this helps us ensure that we have a common understanding of our respective expectations.

By agreeing to this Recording Release and Authorization Agreement you, the presenter, hereby grant to SIAM the right and permission to record, use, publish, stream, podcast, or otherwise distribute any audio, video, materials, or interviews captured as a result of the event at which you are a speaker. Such right and permission includes, but is not limited to the use of, your name, recorded voice or video, photograph, biographical information, handouts or any material based upon or derived therefrom.

You agree that you understand that SIAM may, at its sole discretion, produce presentations or publications based in whole or in part upon video or audio recordings or photographs of said presentation, and that such media or transcripts may appear in print, online, or in any manner or media.

You agree that you understand that SIAM may have an outside company capture the audio or video recording at the event.

You also waive and agree that you have no right of approval, or any claim for compensation. 

You hereby warrant and represent that you have the right to enter into this agreement and to grant the rights granted to SIAM herein. You agree that during the course of the presentation you have not violated the rights of any third parties, including but not limited to copyrights, rights of privacy, trade secrets, and non-disclosure agreements. 

If you need your form countersigned by a manager or supervisor, please complete the attached form, making sure to include the conference name. You can scan the signed form and send to Wendy Carroll or fax and send to 215-382-2220. If faxing please send an e-mail to Wendy Carroll informing her it is being faxed.
